GRAFTON, WI — Sarah LaDuke, MSW, APSW, Community Services Navigator for Ozaukee County, is now located in Suite M, within the Sirona Recovery office on the Ozaukee Nonprofit Center (ONC) campus at 2360 Dakota Drive in Grafton.

The Community Services Navigator program, funded by United Way of Northern Ozaukee, is a collaborative effort between Advocates of Ozaukee, Ozaukee County Human Services, Lakeshore Regional Child Advocacy Center, and Sirona Recovery. The program provides low-barrier, trauma-informed support to individuals and families navigating housing instability, mental health challenges, substance use, and other life circumstances.

“Sarah plays a vital role in helping people connect with services that support safety, wellness, and independence,” said Barb Fischer, Executive Director of Advocates of Ozaukee. “This collaboration ensures care is coordinated across agencies, and we’re grateful to Sirona Recovery for providing space that supports this important work.”

Sarah’s work includes:

  • Helping individuals identify needs and strengths
  • Creating personalized action plans
  • Connecting people with local resources and support services
  • Offering advocacy and consistent follow-up
  • Supporting long-term stability and independence

Friday | 9 a.m.–12 p.m.
